• Application for Invalidity Benefit: This form is filled in by a member who wants to apply for invalidity benefit

• Claimant Particulars Form: The claimant particulars form is used by a member to claim for a benefit. Once you download the form, fill in as required and go to your nearest NSSF office to be taken finger prints. The form must be handed in by the applicant in person.

• Survivor Benefit Application Form: This form is filled in by a beneficiary whose member has passed. The application form is FREE and the claim is processed FREE.
